Errors in "Transputer Development System second edition".

1) In appendix C the following procedures (which used to be in D700D)
should be deleted:

    keystream.from.B004.link
    scrstream.to.B004.link
    terminate.server
    ss.scrstream.from.fold

and this should be added:

    ks.keystream.from.fold  (ssinterf)

2) Page 272 and index

    PROC ks.keystream.from.fold is wrongly named ss.keystream.from.fold

3) The following procedures are in the library ssinterf not streamio as
stated:
